mutable/filenode: Modify mutable filenodes for use with MDMF

In particular:
    - Break MutableFileNode and MutableFileVersion into distinct classes.
    - Implement the interface modifications made for MDMF.
    - Be aware of MDMF caps.
    - Learn how to create and work with MDMF files.
